An Absentee/Mail Ballot Application is attached below for download. It is also available for pick up at Apple Creek School.
When completing an application for an absent voter’s ballot, the application must indicate an allowed type of North Dakota ID. The acceptable forms of identification for voting absentee include the following: (NDCC 16.1-07-06):
- North Dakota form of identification listed for voting in-person.
- Military ID – only for uniformed service or family member stationed away from North Dakota residence.
- Passport – only for North Dakota residents outside the United States.
- Attester – an applicant without a valid form of identification may use an attester. The attester must provide his or her name, ND driver’s license, non-driver’s or tribal identification number, and sign the absentee/mail ballot application form to attest to the applicant’s ND residency and voting eligibility.
Upon receipt of an application properly filled out and duly signed, the school business manager will send by mail: one official ballot, a set of voter instructions, a secrecy envelope, and a return envelope.
